I think this depends on the person. Just as I can only speak for one trans person, I can only speak for one person with anorexia (though the analogy is imperfect because being trans is not a mental illness like anorexia is!). If I could make an assumption, it is that some kind of dysmorphia (as opposed to dysphoria about reality) is what unites a lot of anorexic people - whether it is the delusion that one is fat or the delusion that they are normal.
As for the latter question - yes, that is true! Being underweight prevented me from developing much breast or hip tissue, and even stopped my menstrual cycle, which was very validating to me (and still would be if I hadn't had a hysterectomy by now).
